I give Southwest College of Naturopathic Medicine (SCNM) permission to post my practice information (above) on the SCNM web site. I understand that I am responsible for contacting SCNM with any contact/address information updates. I further understand that if I would like my information removed from the SCNM web site that I must email a request to alumni@scnm.edu. Contact information that is discovered to be incorrect or out of date will be removed at the sole discretion of the SCNM Alumni Office.
